Care Coordinator - Bilingual (English/Spanish) - Part Time
Riverside is seeking a passionate, bachelor’s level Care Coordinator to join our Behavioral Health Community Partners (BH CP) team! This is a unique opportunity to join a growing team of dedicated behavioral health professionals. As a Care Coordinator, the employee will provide person-centered and recovery-oriented care management and coordination to eligible MassHealth consumers served in the community. The Care Coordinator will be responsible for proving outreach to and engaging eligible consumers in services, by assisting consumers to identify their individualized care goals, identify appropriate community-based resources, communicate with the multi-disciplinary team, and completing ongoing need assessments, among other duties as identified.
This is a great opportunity to gain experience with community outreach and providing coordination for consumers with medical and behavioral health needs. The Care Coordinator will also help people served to utilize available community-based resources and maintain a healthy quality of life.
Schedule: Part Time, 20 hours
Pay Rate: $22.95/hourly
- Plus a $.96 language differential!
This is a hybrid position with some remote and in person work required. Periodic travel is also required, within approximately 20 miles of identified office. Offices are located in Needham, Milford, and Lynnfield.

Required Skills
- Experience working with substance use disorders preferred
- Experience with MS Office, Electronic Health Records, and email strongly preferred
- Valid drivers’ license and regular access to reliable transportation required
- Bilingual (English/Spanish) required
Required Experience
Candidates must have one of the following:
- Bachelor's degree in a behavioral health or social service field.
- LADC II Licensure
- Associate's degree in a behavioral health or social service field with one year of professional or lived experience.
- High School Diploma/GED with two years professional or lived experience.
