SUMMARY
Under general supervision provides clinical pharmacy support to clinicians and staff at assigned medical practice-Plymouth. Is responsible for optimizing patients’ health outcomes through Collaborative Drug Therapy Management (CDTM), direct patient care, drug information consultation, academic detailing, cost-savings/quality prescribing initiative rollout, and clinician/staff education at practice(s) of responsibility. Is responsible for central clinical pharmacy support through participation in committees and workgroups, participation in and/or management of standard department programs and services, and participation in the development of organization-wide drug therapy management materials and tools. Schedule for this position is hybrid of onsite and work from home: 3 days onsite per week in Internal Medicine at the Plymouth location and 2 days remote.
