MAP Specialist - ACCS
ACCS Integrated Teams provide clinical interventions and peer and family support to facilitate engagement, support functioning and maximize symptom stabilization and self-management of individuals and to assist individuals in developing skills, establishing natural supports and resources to live successfully in the community.
The MAP Specialist will work in support of all program staff as a member of a Persons’ Team having primary responsibility in delivering interventions identified in the treatment plan with clinical guidance from the Integrated Team clinical staff. He/She/They may develop specialty roles within the team including providing expertise in wellness activities, community engagement, and fostering interest in employment or education.
In conjunction with the Integrated Team, the MAP Specialist will:- Serve as a liaison between the program and Genoa and other select pharmacies
- Track and support all staff in successful progress through the MAP Certification journey
- Provide individualized training and one to one support as needed for staff in utilizing the MAP system.
- Write, review and submit Medication Occurrence Reports (MORs) and Incident Reports
- Develop relationships with providers to assist with obtaining signatures on med orders
- Assist with smooth transitions from other levels of care, including internal Riverside Respite and CCS programs by providing assistance with medication orders
- Develop and implement a standard process across all GLEs for med administration
Schedule: Full Time, 40 hours
Pay Rate: $26.43/hourly
