OBJECTIVE/PURPOSE:
Under the direction of the Shared Living Program Manager, the Shared Living Program Coordinator provides administrative and clinical supervision to staff working with consumers with intellectual disabilities who are in home placements as well as general oversight to the Shared Living Program. The coordinator will assess, plan, implement, monitor, evaluate the needs of consumers and responds accordingly. The Shared Living Program Coordinator ensures that the services provided through Shared Living meets the needs of the consumers, ensures that services are delivered with dignity and respect and meet regulatory requirements. Assists case managers to problem solve and find solutions to issues and crises.
MAJOR RESPONSIBILITIES:
- PROGRAM DEVELOPMENT AND SUPERVISION
Provides administrative and clinical supervision to Case Managers. Monitors quality of services provided. Assists Case Managers to establish meaningful treatment plan (ISA) goals. In conjunction with the Shared Living Program Manager and Human Resources, selects, trains, motivates, and supports staff. Conducts annual performance evaluations. Conducts periodic in-service training, as appropriate. Assists program managers and staff in developing creative programs. Interviews, recruits, and trains new Shared Living (SL) providers. Participates as a member of the DS Emergency On-Call System. Assists with recruiting and finding new placements for clients and may assist with housing inspections. May assist with planning client moves. Ensures all monthly home provider vouchers are submitted timely and accurately.
2. QUALITY ASSURANCE AND COMPLIANCE:
Ensures that programs and documentation meet regulatory compliance. May serve as QDDP for consumers in the Shared Living program, if meets requirements. Facilitates programmatic changes to meet the needs of the consumers. Reviews data and provides feedback to managers and the individual's team. Reviews and approves staff's required agency documentation, as appropriate. Assists managers in ensuring that waiver budgets match the services provided to the consumer. Facilitates housing inspections as per requirements. May provide case management services as assigned. Provides training to home providers as needed.
3. NETWORKING AND CONSULTATION / EDUCATION
Serves on agency committees, as assigned or appropriate. Assists case managers in the coordination of services, as requested. Provides assigned training as requested or appropriate. Represents the division on statewide task forces as assigned. May contribute to community awareness through the media or speaking engagements. Maximizes the division's reputation when interacting with other entities.
4. INFORMATION MANAGEMENT, RECORD KEEPING AND FISCAL MONITORING
Reviews various clinical and administrative forms for billing, management information and clinical purposes. Prepares reports, as requested. Reviews management and outcome reports completed by managers. Reviews waiver budgets, as appropriate and advises Division Director of necessary changes. Ensures the timely completion of documentation from Case Managers. Assists case managers to monitor contracted budgets through the fiscal intermediary.
Qualifications:
EDUCATIONAL REQUIREMENTS:
Bachelor's degree preferred but relevant experience will be considered. Familiarity with all programs in Microsoft Office, outlook, and EMR helpful QDDP eligible
