Major Job Responsibilities
DIRECT CLINICAL SERVICES: The School Based Clinician provides individual, family or group counseling services to parents and children in the community or school, as needed. The School Based Clinician will identify the mental health and behavioral needs of students; develop and implement treatment plans; conduct clinical interviews, assessments, observations, and complete clinical reports; review referrals of students who need intensive mental health services; provide support in the development of behavior support plans; conduct crisis and/or risk assessment and response, as needed.
CASE MANAGEMENT / SERVICE COORDINATION: The School Based Clinician will make home visits to support families, as needed. Facilitates communication between agencies on behalf of the student. Attends team meetings to plan and monitor services. Provides case management, collateral contact and social support, as needed.
CONSULTATION/EDUCATION SERVICES: The School Based Clinician will provide consultation services to teachers and school personnel, as needed. Consultation can be educational or evaluative.
CLINICAL RECORD-KEEPING: The School Based Clinician initiates and maintains, in a timely and thorough manner, the wide variety of record keeping and administrative documentation required to account for services provided to consumers - including, but not limited to, consumer specific assessments, case services, progress notes, time sheets, schedules, billing reports to third party payers and assigned periodic reports. Follows all UCS record-keeping procedures and performs other duties as necessary.
ADMINISTRATIVE: The School Based Clinician attends all UCS and community meetings necessary to promote coordination and communication. Follows all UCS record-keeping procedures and performs other duties/projects, as necessary. The school-based clinician is required to meet or exceed the billing case rate as defined under the Success Beyond Six contract.
REQUIRED QUALIFICATIONS:
- Master's Degree in a related clinical field.
- Licensure, or on track for licensure, in Vermont as a Clinical Mental Health Counselor, Social Worker, or Psychologist.
- Experience in providing services to children and families preferred.
- Valid driver's license.
- Window skills, both typing and navigating, e-mail and excel usage.
